1.1 Form DSP-122 - complete only by the principal applicant.
1.2 Form DS-230 Part 1 and 2 - complete by principal applicant and all family members applying for the Diversity Visa. Note, each form DS-230 part 2, Sworn Statement should be submitted unsigned.
