It is acceptable to use correction fluid to correct mistakes made while completing your forms. If you have detected an error on a form you have already submitted, you can send the corrected information to KCC through regular mail or email. If there are more than one or two minor corrections, you may submit a corrected copy of the forms. You can use photocopied forms or you can download them from
http://www.travel.state.gov/visa/frvi/forms/forms_1342.html.
KCC does not have the ability to retrieve or make changes to the data that was provided on the original entry to the lottery.
Please complete the forms DS 230 Part I, DS 230 Part II, and DSP 122 with the correct information. Once received at KCC, the information will be included in your case file for review.
Failure to submit the requested photos will not delay the processing of your case. Though, the photos are required. It is acceptable to submit the photos separately to KCC. When doing so, please remember to lightly print the name of the subject and the case number on the back of the photos with a felt pen to avoid mutilation of the photos. The photograph should be attached securely to the appropriate DS-230 form with either tape or staple.
If you are scheduled for an interview, you may also provide the photos at the time of interview.
Both typed and clearly written responses are acceptable.
Please use black or blue ink only if you choose to hand write on your forms.
Document Requirements
Obtain the original documents or certified copies from an appropriate authority for yourself and each family member who will accompany you to the United States. All documents that pertain to your petition are required to be presented at the interview.
Birth Certificates
Obtain the original, or certified copy, of the birth record of each family member (yourself, your spouse, and all unmarried children under the age of 21). The certificate must contain the:
· Person’s date of birth
· Person’s place of birth
· Names of both parents, and
· Annotation by the appropriate authority indicating that it is an extract from the official records
Unobtainable Birth Certificates
Your birth record may not be obtainable. Some reasons are listed below:
· Your birth was never officially recorded
· Your birth records have been destroyed
· The appropriate government authority will not issue one
Please obtain a certified statement from the appropriate government authority stating the reason your birth record is not available. With the certified statement you must obtain secondary evidence.
For example:
· A baptismal certificate that contains the date and place of birth and both parent’s names providing the baptism took place shortly after birth
· An adoption decree for an adopted child or
· An affidavit from a close relative, preferably the applicant’s mother, stating the date and place of birth, both parent’s names, and the mother’s maiden name.
Note: An affidavit must be executed before an official authorized to take oaths or affirmations. Information regarding the procedures for obtaining birth certificates is usually available from the embassy or consulate of the country concerned.
Translations
All documents not in English must be accompanied by certified English translations. The translation must include a statement signed by the translator that states that the:
· Translation is accurate, and
· Translator is competent to translate.
