I think the "modern" blues has borrowed a lot from other music styles - everything is connected somehow. For instance Robben Ford is a great jazz-player as well, SRV took a lot of Hendrix style and if You look at Guthrie Govan he's incoorperating just everything about modern guitar. Also it happened a lot tonally-wise, since the "old" blues was mostly based on the blues scale and occasionally the major third. Nowadays, players use a lot of other stuff in their solos like Mixolydian modes, chromatism, playing on each chord instead of playing one scale for the whole progression. Traditional blues is a blend of musical forms and inlfuences that existed in those old days. Modern blues is all that + influences of modern age. So modern blues is evolved but still kept his roots
Early Blues. (Acoustic)
Derived from the Negro Spirituals sung during the days of Slavery.
Mix these spirituals with a more European Folk tradition and you get your basis of a 12 Bar pattern.
As the now-free Slaves move around the North American Continent they take this music with them.
It now starts to incorporate the Regional Music of whichever area.
As performers ply their trade on street corners and slide players find they don't have the volume to compete against standard Acoustic Instruments the 'Resonator' is invented.
Early Electric Blues.
As Instruments start to be amplified there is a definite trend to different types of Blues.
Delta Blues in the Deep South, Texas Blues (mixing Blues, Country and Rock and Roll 'Shuffle Beats') and in the North 'Chicago Blues' (a mix of Blues, Trad Jazz and Big Band Swing).
Modern Blues.
A mixture of everything previously mentioned and now incorporating elements of Modern Jazz.
