Sunderland won the Championship in Roy Keane's first season in charge after a 5-0 victory over relegated Luton.
Keane, who won seven Premiership titles as a player with Manchester United, took charge of a struggling Sunderland in August when they were bottom of the table having lost their first four games.
Birmingham, who were top of the table at the start of the day, slumped to a 1-0 defeat at Preston North End but are also promoted to the Premiership having finished second.
The Black Cats got off to the perfect start with Anthony Stokes and Daryl Murphy giving them a 2-0 half time lead. Stokes then set up Murphy to make it 3-0 before Ross Wallace and David Connolly finished the match in style for Sunderland.
Birmingham meanwhile missed a number of second half chances before being undone by a scrappy Simon Whaley winner in the 85th minute. Derby, who had already secured third place, beat the already relegated Leeds 2-0 at Pride Park.
West Brom slaughtered a wayward Barnsley 7-0 to book their play-off spot. First half goals from Kevin Phillips, Nathan Ellington (two) and Robert Koren meant the contest was as good as over by the half time interval, before Phillips snatched another two to complete his hat-trick. Zoltan Gera also got his name on the score sheet to finish the rout for the Baggies.
Wolves snatched fifth place after recovering from a going a goal down to beat Leicester 4-1. Ian Hume gave the home side the lead after just three minutes but goals from Sevi George Olofiniana, Michael Kightly and Andy Keogh combined with an own goal from Gareth McAuley gave Mick McCarthy's team the win.
Southampton were also forced to recover from losing an early goal before they secured the final play-off spot with a 4-1 win over Southend.
The Shrimpers knew they were relegated going into the match but Adam Barrett gave them hope of a final win with a left foot volley 13 minutes into the match. But Kenwyne Jones levelled the scores before half time before Leon Best and Jones wrapped up the win in the second half.
Stoke saw their play-off hopes dashed by a 1-1 draw at QPR as did Colchester who were beaten 2-0 by Crystal Palace at Layer Road.
In the day's other games, Hull, who knew they were safe after Leeds were docked 10 points for going into administration, lost 2-1 to Plymouth, Ipswich won 3-1 against Cardiff while Jermaine Johnson, Deon Burton and a Dickson Etuhu own goal gave Sheffield Wednesday a 3-2 win over Norwich.
