A fairytale ending not just for Italy, but for Giuseppe Rossi who scored twice to help the Azzurri to a 3-1 victory over the country of his birth.
Landon Donovan had given the USA the lead in a powder keg of a match in which Italy had a goal strangely disallowed.
Italy and the USA last met in a hot-headed group match at the World Cup in Germany three years ago, which ended 1-1.
Three players were sent off and some feared tempers would flare up once again this time round.
Alberto Gilardino, the man who scored that day in Kaiserslautern, led the line for Italy in an offensive 4-3-3 formation.
He got the nod in favour of Luca Toni after scoring twice in a 4-3 victory over New Zealand last Wednesday.
Chances were few and far between in the opening 15 minutes, although Italy played some neat football around the box.
The Azzurri looked to control possession and give the ball to la regista in chief Andrea Pirlo, who orchestrated some early moves.
The first real chance of the game fell to Nicola Legrottaglie, who got on the end of a Pirlo free kick after 20 minutes.
The ball bounced in front of him in the six-yard box, he dived to meet it and headed it inexplicably wide before scampering back up the pitch in shame.
Opportunities started to come thick and fast after that, as Howard struggled to hold a Pirlo free kick and corner.
The USA were growing frustrated and it didn't take long for the temperature to come to the boil.
Ricardo Clark lunged in late on Rino Gattuso after 32 minutes, kicking out a leg, and got sent off for his trouble.
Italy should have gone 1-0 up soon afterwards after Jonathan Bornstein kicked the ball into his own net.
However, Mauro Camoranesi was flagged for offside and the goal was disallowed.
To make matters worse, Giorgio Chiellini gave away a penalty five minutes before half time and Donovan found the corner, beating Buffon to put the USA in front.
Marcello Lippi tried to change things just after the interval, bringing on Giuseppe Rossi for Gattuso and Riccardo Montolivo for Mauro Camoranesi.
Rossi made an immediate impact, scoring a screamer against the country of his birth just two minutes later.
The New Jersey born midfielder picked up the ball in USA territory, ran towards the box and unleashed a rasping shot that gave Howard no chance.
Just 10 minutes later, De Rossi put Italy ahead with a speculative shot from 35 yards out, which bounced in front of Howard and found the right hand corner.
Clint Dempsey nearly got the USA level with 10 minutes to go with a shot from distance, but Buffon merely used it to warm up his hands.
Italy kept looking for a third and found one in the 92nd minute after Pirlo danced down the left and found Rossi, who volleyed past Howard.

Rossi dedicates goals to family Monday 15 June, 2009
Giuseppe Rossi has dedicated the two goals he scored in Italy's 3-1 Confederations Cup victory over the USA to his family back in America.
The 22-year-old, who grew up in Teaneck, New Jersey, came off the bench after 56 minutes and made quite an impact.
Italy were 1-0 down only for Rossi to get them level two minutes after coming on with a belting shot from 30 yards.
He then secured the points with a second deep in injury time, half volleying a gorgeous pass from Andrea Pirlo past Tim Howard.
“I am happy for the result and for the double,” Rossi said after the game. “I dedicate my goals to my family who are watching the game on TV in America.
“When I came on the team were suffering. The USA are tough. They run and fight a lot, but in the second half they were tired.
“Now we can have a great tournament,” Rossi concluded.
Italy now lie second in Group B behind Brazil, who edged out Egypt 4-3 earlier today.
Lippi hails World Cup spirit Monday 15 June, 2009
Italy Coach Marcello Lippi has praised his players for the heart they showed in coming back to beat the USA 3-1 in the Confederations Cup.
The Azzurri went into the break 1-0 down after LA Galaxy midfielder Landon Donovan scored a penalty after 41 minutes.
Giuseppe Rossi drew Italy level early in the second half and Daniele De Rossi put them ahead after 72 minutes.
Rossi popped up again in injury time to get his second and Italy's third with a beautiful half volley.
“We played our cards as we always do,” Lippi told Rai. “While the USA had their vivacity and their running.
“But it was strange at the beginning of this game. We played little, perhaps because of the penalty, but then again we had a numerical advantage.
“Then in the second half we started to play,” Lippi said.
“I am happy Rossi scored two goals. We can play good and bad, we can even get a game wrong, as we did at the beginning today, but this team has the heart of that which won the World Cup,” he concluded.
